How to Order an Additional Plaque

Purchase each award you would like to order. You may include all awards in a single transaction within this system.

To ensure you are selecting the correct award (Merit, Special Achievement, Honor,) please see list below. 

After completing your purchase, you will receive a confirmation email with instructions to finalize your order. This step includes completing a PDF form with the exact wording and details you would like on the plaque, which you will then upload to Dropbox as directed in that confirmation e-mail.
